Student Practicum Evaluation

Each student will submit an evaluation of their practicum experience, when they complete their practicum hours. The Student Evaluation must contain ALL of the following information:

	Name of the organization where you completed your hours;
	Total hours completed;
	Summary of duties performed at each agency;
	A brief history of the agency (when was it established, how many staff, mission statement, etc.)
	Evaluation of Practicum Goals -- did you accomplish your goals listed in the 1st Assignment?
	The relationship between your goals and the actual practicum experience;
	Describe one thing you "enjoyed" and one thing you "disliked" about working for one of the agencies:
	Describe one "interesting" experience from your practicum hours;
	Would you like to work for the agency as a regular employee? Why or why not?;
	What impact has the practicum experience had on your career goals?

Important Note: All 240 hours must be completed AND all reports must be submitted by Friday, December 14 by 5:00 PM. There are no "Incompletes" or extensions.
